Good morning - As you know, the Florida Division of Emergency Management (FDEM) is in the process of developing standardized forms for Cat A/B claims.  They would like input on the forms from affected parties and will likely be sending out various information requests as they go through the process of finalizing the forms.  To start, they have asked interested parties to fill out the brief survey below - they are particularly interested to hear back from the electric utility sector so please respond to this survey by the March 11 deadline.  There will be further opportunities for public input on the forms - we will keep you informed about next steps and any new data requests from FDEM.
